Patient safety and managing risk is an essential component of health care. This book manages to capture the essence of this topic in an accessible and easily applied way. It covers a lot of ground and the activities provide the reader with an opportunity to reflect on their own practice. As a health care professional within education I will be recommending this to students. This book fills a gap in the market, is easy to read and apply to practice.

Human Factors in patient safety: A long overdue commonsense approach to healthcare which should have been implemented a long time ago. It is good to see that the check list approach of the aviation industry is at long last having an impact on healthcare delivery.

This is an excellent book that is well written and researched. It is part of ‘Transforming Nursing Practice’ series which, the ones I have read are very good, relevant to todays caring environment and full of useful information that can be taken into practice. Each chapter commences with its aims and where the topic fits into the NMC Standards and Essential Skill Clusters. Within each chapters there are also activities based around critical thinking, reflection and Evidence Based Practice and Research, three key elements. In addition there is also a glossary of essential terms, web sites to visit and additional reading. Very useful text for the nurse and all other health care practitioners.
